There are changes to the Medicaid level eligibility criteria for enrolling in most Peoples Health D-SNPs:

Medicaid level and parish of residence determine plan eligibility.

Existing D-SNP members: Members will be crosswalked to the appropriate contract number and segment (based on Medicaid level and parish of residence) for Jan. 1, 2027. Members will move to either:

• A new segment of their existing D-SNP or
• Another Peoples Health D-SNP appropriate to their Medicaid level

In either instance, current members do not need to complete a new enrollment application.

Prospective D-SNP members: Medicaid level and parish of residence determine plan eligibility. Use the eligibility tool and Medicare Product Portal in Jarvis to determine the appropriate plan.
